Your First Appointment
If you're a new therapy patient, please complete the following forms before your first appointment. It saves time, and helps us understand your current physical well-being, enabling us to give you the best, most informed care.
Forms can be completed in Acrobat Reader. Once finished, you can save and email them to us, or print and bring them to your first appointment. (If you prefer, you may also print and complete them by hand.)
Please note: If you chose not to complete the appropriate forms in advance, arrive at least 30 minutes before your first appointment.
You should also bring:
- Insurance card
- Doctor’s referral (if needed)
- Appropriate clothing*
- Any operative reports, diagnostic tests (x-rays, MRI, etc.) your physician has given you
- Any questions you might have
Your therapy sessions will vary from 30 to 60 minutes. We make every concession to have you see the same physical therapist team member each time. On the rare occasion that this isn’t possible, another one of our trained physical therapist team members will cover your appointment(s) as needed. Less than 10% of our patients see more than one physical therapist member for treatments.
Mettler charges a $50 fee to clients who fail to show up for an appointment without calling ahead. If you’re late to an appointment, you may not have your full session time. However, if your therapist is behind schedule, we’ll make every effort to allot your full session time.
*Appropriate clothing: Wear comfortable clothes. If you have a problem with your lower back or below it, shorts are helpful. For women who have problems with their back (and any area above), sports bras are useful. If you forget, or if acquiring the clothing is an issue, we’ll provide shorts and gowns to accommodate and respect your modesty and privacy.
