your life as well. Patient Information FormsIf you've scheduled an appointment for a free consultation, you may fill out our Physical Therapy Consultation Form before you come in. This will help the therapist learn more about your condition and allow more time for the initial evaluation. If you're a new therapy patient, you can print and complete our New Patient Information Form and Patient Information Form prior to your first appointment. This saves time at your first appointment, and also helps us to understand your current physical well-being, which will enable us to give you the best, most informed care. Also, please read and sign our Policies on Fees and Insurance Claims, Notice of Privacy Practices, and HIPAA Release. All Mettler patients are required to read and sign these forms before treatment, so it would be helpful to complete these forms before your first appointment. DirectionsMettler has two convenient full-service locations, to serve both our campus community and the surrounding areas. Arriving from Interstate 74 to the Crossing:From Interstate 74, take the Prospect Avenue exit south to Windsor Road, turning west (right) there. Follow Windsor Road west until you reach the Mettler Center at 2906 Crossing Court, which will be on your right. Arriving from Interstate 72 to the Crossing:From Interstate 72, take Mattis Avenue south until you reach Windsor Road. Turn west on Windsor (right), and follow it until you reach the Mettler Center at 2906 Crossing Court, which will be on your right. Arriving from Interstate 57 to the Crossing:From Interstate 57, take the Curtis Road exit (#232) until you reach Duncan Road. Turn north on Duncan (left), and follow it just past Windsor Road until you reach Crossing Court. Turn east on Crossing (right), Mettler Center is located at 2906 Crossing Court, which will be on your left. |
